VIEW AND DOWNLOAD OF MEDICAL REPORTS
From this page you can access the radiological and laboratory reports of exams made at the Santa Lucia Foundation. Each report is available online for 30 days from the date indicated on the report collection form, which was delivered to you by check-in at the day of your exam (attention: before this date User ID and Password will not work).
To enter the Medical Report Area you need the User ID and Password communicated to you by check-in. If you no longer have them available, you must go to the Reception Office of our Outpatients Clinic (via Ardeatina 306, Rome), to obtain new login credentials (please, bring with you a valid ID).

REQUEST OF CLINICAL RECORD
Medical records may be obtained from the Admissions Office (from Monday to Friday, h. 8:30 – 13:00, 14:00 – 17:30; Saturday, h. 8:30 – 12:30). Payment can be made in cash or Debit and Credit card. If someone other than the patient is requesting medical records, the proxy must provide: 1) delegation document signed by the patient authorizing the delegate to obtain the records; 2) copy of the patient's identity document; 3) original identity document of the delegate.
Time and Costs
The hospital strives to provide the medical record within 30 days from the request, mailing it to the address indicated by the applicant. The cost is € 30,00 for any medical records related to hospitalization (inpatients and day hospital) and € 15,00 euro for any medical records relating to Extensive Rehabilitation Outpatient care.
Importance of Your Medical Records
The medical record is a public document that has clinical value and can help evaluate the effectiveness of care and effective disclosure of information activities between heath care professionals. Medical record data may be used anonymously for statistical and scientific surveys as well as medical-legal and economic-administrative purposes. Since medical data include sensitive health related data, it is therefore subject to the laws for the protection of privacy.
